Pinoko experiences recurring chest pain resembling a heart attack. Black Jack conducts thorough medical examinations but finds no physical abnormality. During this period,Black Jack receives an urgent medical request from a former patient, Yurie Saionji. Yurie reveals that ten years earlier, Black Jack removed a teratogenous cystoma from her body. Black Jack reconstructed that cystoma into Pinoko, who has lived as his assistant ever since. Yurie explains that the Saionji family carries a curse where one of any twin pair dies within five years. To avoid this fate, her twin sister grew inside her own body as the cystoma that became Pinoko. Yurie must perform a traditional dance ritual every decade to maintain the curse's seal. The upcoming ritual requires her to remove the cystoma remnant permanently. Black Jack agrees to the operation, separating Pinoko's biological origin from Yurie. Yurie survives the procedure and prepares for the ritual, but she weeps over losing the sister who lived within her. Pinoko remains with Black Jack, now knowing the full truth of her unnatural birth. The government of a war-torn country kidnaps Black Jack and forces him to treat their dying dictator. The dictator suffers from a brain tumor,and Black Jack performs the surgery under armed guard. A female guerrilla fighter named L infiltrates the compound seeking revenge against the regime. She reveals that the dictator murdered her family and destroyed her village. Black Jack completes the operation successfully, saving the dictator's life. L captures Black Jack and demands he kill the dictator instead. Black Jack refuses to take a life, having just saved it. He expresses rare regret for saving the dictator, knowing the man will continue his brutal rule. L releases Black Jack and carries out her revenge alone. The episode ends with Black Jack leaving the country, haunted by his role in preserving a tyrant's life.
